4. Valid Philippine Passport
5. 2 Primary IDs Government Digitized Issued (SSS, PRC, Driver’s License, New Postal ID, UMID ID, Barangay ID)
6. Secondary ID if only 1 Primary is available please provide another 2 of the following (Voter’s ID, Postal, PhilHealth, TIN, NBI, Police Clearance, Company ID w/ COE, Laminated Brgy. ID & Senior Citizen ID)
7. Latest Proof of Billing (If not under clients name please provide us additional Brgy. Clearance)
8. Tax Identification Number & SSS no.
9. If you are applying for student visa we need any proof that you are applying as student. (Offer/Acceptance Letter or email thread (hide confidential))
We have 3 days application:
1st day: Submission of requirements and payment (personal appearance)
2nd day: Opening of Account (Passbook savings account only)
3rd day: Request and release of bank certificate, payment of 300.00 (shouldered by account holder.
